What is a PRI and SCREEN assessment?
Patient Review Instrument (PRI) is an assessment tool developed by the New York State Department of Health to assess selected physical, medical and cognitive characteristics of nursing home residents, as well as to document selected services that they may receive.
The SCREEN assessment is a tool used to identify current and/or potential mental illnesses, developmental and/or intellectual disabilities.
Why do we need a PRI and SCREEN assessment?
New York State requires that PRI and SCREEN assessments are to be completed prior to admission to any long-term care facility.
Where are these assessments performed?
Assessments are performed wherever your loved one is most comfortable. Assessments are typically performed at your loved one's home or assisted living facility.
What do I need to bring to the assessment?
Please bring a list of current medications and any recent medical records.
What is the cost?
Completed PRI and SCREEN assessment cost is $199.
How soon can the PRI and SCREEN assessments be done?
Most assessments can be scheduled and completed in 2-3 business days.
